US tubal patency

The female reproductive organs are closely connected to ensure fertility. The fallopian tubes serve as the pathway for a fertilized egg to travel from the ovary to the uterus, where it can implant and grow. If one or both tubes are blocked, the egg may not reach the uterus, potentially resulting in an ectopic pregnancy.

To diagnose such conditions, a fallopian tube patency test — known as sono-hysterosalpingography (Sono-HSG or echohysterosalpingography) — is recommended.

The safest and most effective method is ultrasound-based tubal patency testing (also called echohydrotubation). Unlike standard pelvic ultrasound, fallopian tubes are not normally visible. With this method, a contrast solution is introduced to make the tubes visible in real time using ultrasound.

Indications and Contraindications

This test is indicated in cases such as:

  • Irregular or absent menstrual cycles
  • Pelvic or uterine pain
  • Suspected reproductive tract infections

It is not performed if the patient has:

  • An existing pregnancy
  • Uterine bleeding
  • Acute inflammation with severe pain

Preparing for the Procedure

Before the test, patients must:

  • Undergo blood tests for infections
  • Complete a vaginal swab test for bacterial flora
  • Be examined by a gynecologist for any inflammatory diseases

The ultrasound is typically scheduled between days 5 and 10 of the menstrual cycle, when the cervical canal is most open, allowing for easier fluid movement.

How the Procedure Is Performed

Many women are familiar with X-ray hysterosalpingography, where contrast dye is introduced and radiation is used. However, IPF Clinic offers a safer, radiation-free alternative using ultrasound.

Here’s how the procedure works:

  1. The cervix and vagina are cleaned
  2. A small catheter with a balloon is inserted into the cervix
  3. A saline-based contrast fluid is gently introduced into the fallopian tubes
  4. As the fluid flows, its movement is tracked in real time via ultrasound
  5. The doctor observes the filling of the tubes, checks for blockages, adhesions, or abnormalities
  6. In some cases, the pressure of the fluid can clear minor adhesions, increasing the chance of natural conception

If the fluid successfully flows from the uterus through both tubes, patency is confirmed.
