
Transvaginal ultrasound
Transvaginal ultrasound is a safe, highly informative diagnostic procedure in which a special ultrasound probe is gently inserted into the vagina to examine female reproductive organs. This method offers significantly better accuracy and imaging quality than traditional abdominal ultrasound, as it allows for close proximity to the uterus, ovaries, and fallopian tubes.
There are no contraindications, though this method is only performed on women who have been sexually active. For younger patients or virgins, transrectal ultrasound (via the rectum) may be used if abdominal ultrasound does not provide enough information.
When Is a Transvaginal Ultrasound Needed?
A transvaginal ultrasound can be done for diagnostic or preventive purposes. Your doctor may recommend the procedure if you are experiencing:
- Lower abdominal pain
- Enlarged uterus or ovaries (detected during a pelvic exam)
- Suspected intrauterine or ectopic pregnancy
- Abnormal bleeding during or after your period
When Should It Be Performed?
Timing depends on your menstrual cycle and the condition being evaluated:
- For uterine fibroids, ultrasound is done in the first half of the cycle
- For endometriosis, it’s done in the second half
- During pregnancy, it is typically done between 11–14 weeks

How the Procedure Works
- You’ll lie down on an examination table
- A protective cover is placed over the probe and ultrasound gel is applied
- The probe is gently inserted into the vagina
- The procedure is painless, though some discomfort may occur if active inflammation is present
- The exam lasts about 20 minutes, with mild, non-painful pressure
Preparing for a Transvaginal Ultrasound
No preparation is needed if:
- The exam is for routine evaluation of the uterus and ovaries
If you are pregnant:
- Drink about 500 ml of water one hour before your scan to ensure the bladder is full
Additional recommendations:
- Avoid gas-producing foods (e.g. sweets, dairy, bread) a few days before your appointment
- If you are prone to bloating, take anti-gas medication
- No enema is required
- Alert your doctor if you are allergic to latex
