What Is Placentocentesis?

Placentocentesis is an invasive prenatal diagnostic procedure performed in a clinical setting to detect or rule out genetic abnormalities in the fetus.

The procedure involves using a fine needle, under ultrasound guidance, to take a small sample of placental tissue through the abdominal wall. This tissue is then cultured in a laboratory. After about a week, the fetal karyotype (chromosomal analysis) can be evaluated.

Placentocentesis offers a high level of accuracy and relatively quick results. However, it carries a slightly higher risk of complications compared to amniocentesis.

When Is Placentocentesis Performed?

The placenta is fully formed by 16 weeks of pregnancy. Before this point, chorionic villus sampling (CVS) may be used instead. However, the earlier an invasive test is done during pregnancy, the greater the potential risk.

The latest recommended time for placentocentesis is 20 weeks, to allow enough time for results to be received before 22 weeks of pregnancy.

Indications for Placentocentesis:

  • High-risk results from first or second trimester prenatal screening
  • Family history of genetic disorders
  • Severe oligohydramnios (low amniotic fluid), making amniocentesis impossible
  • Parental chromosomal rearrangements or previous child with a genetic condition
  • Exposure to harmful substances early in pregnancy (radiation, toxic drugs, chemotherapy)
  • History of recurrent miscarriage or perinatal loss

Contraindications:

  • Threatened miscarriage
  • Fever or acute respiratory illness at the time of the procedure
  • Exacerbation of chronic diseases
  • HIV or hepatitis (due to risk of fetal infection)
  • Skin infections on the abdomen
  • Technical complications (e.g., adhesions, large fibroids on the front of the uterus)

Note: A negative Rh factor in the mother is not a contraindication, but if the father is Rh-positive, anti-D immunoglobulin must be administered to prevent Rh sensitization.

How Is Placentocentesis Performed?

The procedure is always done under ultrasound guidance:

  1. The patient lies down while the doctor uses ultrasound to identify the safest location for sampling.
  2. A thin needle is inserted through the abdominal wall.
  3. A small piece of the placenta is collected and removed with the needle.
  4. The sample is sent for chromosomal analysis.

The procedure can be uncomfortable or mildly painful. Afterward, patients are advised to rest for several hours either at the clinic or at home, maintaining bed rest. Some mild cramping or tenderness at the puncture site may occur.

Is Placentocentesis Safe?

As with any invasive diagnostic procedure, there are risks — including bleeding, miscarriage, or pregnancy loss. The risk of complications is approximately 2%.
